DATE: September 20, 2012 TO: ador County Healthcare Workers FROM: Bo artman, M.D., ador County Health Officer RE: Mandatory Influenza Vaccination of All Healthcare Workers in ador County

As Public Health Officer of ador County, I am requiring that all ador County healthcare workers (HCWs) receive an annual influenza vaccination or, if they decline, wear a mask for the duration of the influenza season when providing direct patient care within six feet of a patient. Each year influenza infection affects 5 to 15% of the US population. Influenza results in over three million hospitalization days and 31 million outpatient visits yearly. Patients in our healthcare facilities are particularly vulnerable to influenza. Healthcare workers are both at risk for developing influenza and of transmitting the virus to their patients. In your role as healthcare professionals and my role as Public Health Officer, we share the common goal of minimizing the spread of infectious disease. Influenza vaccination of HCWs protects patients and reduces employee absenteeism during influenza season. State law requires that general acute care hospitals and certain employers offer influenza vaccinations to employees. If hospital employees decline vaccination, they are only required to sign a declination statement in lieu of vaccination. While compliance rates with these laws are high, actual HCW vaccination rates are not. Rates may actually be less than required to create effective immunity within the workforce to suppress the spread of infection. Mandatory vaccination or masking policies have been shown to increase HCW vaccination rates to greater than 90%. In addition, pending legislation will require that all hospitals achieve an influenza vaccination rate of 90% by 2015. Our goals are to increase rates of influenza vaccination of all HCWs, reduce employee absenteeism during influenza season, and reduce HCW transmission of influenza to patients, thereby reducing influenza-related mortality and morbidity in ador County. Order I, as the Public Health Officer of ador County, am requiring ador County healthcare workers receive an annual influenza vaccination by November 1 or, if they decline, wear a mask for the duration of the influenza season when providing direct patient care within six feet of a patient. Simple surgical masks will meet the requirement. Duration of Order This order remains in effect from November through March for each successive annual influenza season unless otherwise modified, suspended or rescinded by the ador County Health Officer. For the purposes of this order, the influenza season in California is defined as November 1 through March 31 of the following year. In any given year, if influenza surveillance data demonstrate an unusually late peak with continued widespread influenza activity into the Spring, an extension of this public health order may be necessary. Definition of HCWs For the purposes of this order, “healthcare worker” is defined as a person, paid or unpaid, working in a healthcare setting who provides direct patient care within six feet of a patient. I appreciate your cooperation, assistance, and support in protecting the residents of ador County. If you have any questions, please contact ador County Public Health Department at () 2-07.

I think everyone who doesn't care to submit to this unlawful "order" also wear a V mask in a show of solidarity against Adolf or "the Man". Make the F'n sick patient wear the mask. NOT you if you're not sick. What a load of socialistic crap.

This update just came my way, so I'm passing it along.

"SB 1318 vetoed with this msg: “Encouraging HCW to be vaccinated against influenza is good policy, and I support the national goal of a 90% compliance rate by 2020. .... This bill would move the date up to 2015 and make compliance mandatory, which are requirements I do not believe are reasonable. ...health facilities ... make these decisions on their own.”

In the military we had to have the flu shot, no choice...unless we were allergic or pregnant. Jay's company provides them every year for free. I have to have that and the pneumovax because of my immuno suppresants. I dunno. On one hand yeah, how do you FORCE people to do it. But...the flu is responsible for many missed days of work, and employers like mine have a shit fit over that.

For those that care to do their own research here is my link to the CDC advisory cdc @ service . govdelivery . c om feel free to put in some time and read for yourself, then you don't need to take my word for it. I don't claim to be an expert on any of these topics. I am better informed than most and less informed than some others. I'm here as a resource and a messenger for those who care to take the information I present as a starting point and find out for themselves what is what. Obviously I'm choosing more controversial topics just because it does generate some degree of interest. Some take some pleasure in trying to discredit and neuter the messenger. If that's their level of function they'll be among the first lost when these issues manifest and impact them or a loved one. When the fecal material collides with the rotary oscillator remember someone tried to raise your level of concern and interest.

Sed, and did that study also show the greater effect is achieved by the hand washing (upward of 70-80 % of your cited 60-80%), depending on the mode of transmission. AS to the droplet size certainly the more effective mask which handles the smaller droplet wins. That no contest as it stops a whole lot more than the less effective standard issue cheapie.

Holly,

I know, I wasn't as verbose as others, but I stated the very same things as Cincy, then went on to ask about the mask myself, but like you and majority of others, my question was initially ignored, when I re-posted the question (Yeah, how dare a woman be persistent) he side-stepped answering it.

Sed - I believe in washing hands and I have no problem with health care workers wearing a mask. In fact, you may recall that i had the flu earlier this summer? When I went to see my Dr, she chose to put a mask on (the first time she has ever done so during my appts). She apologized for doing so, worried that I'd be offended, but was leaving for vacation in a couple of days and didn't want to risk catching a flu that had me homebound for 8 days.

I was just trying to point out that despite Always' dismissive answer to Cincy's question, where he essentially argued that the most common masks were minimally effective, he himself wears a mask in the workplace. Of course he failed to answer my question as to what kind of mask, just like he failed to respond to my earlier post, to most of Cincy's questions, and to virtually all posts that offer a reasoned opposing point of view.
